Home Medicines Reviews (HMR) can be incorporated into number of other team care and management plan items:

Suggested Annual Care Cycle for Chronic Disease Patients 

(Using GP Management Plans)

Month 1

Visit 1: Standard health assessment- 703 (referral for HMR as part of this and recommendation that a GP Management Plan be set up for patients with chronic diseases) Fee: $132.70 Benefit: 100% = $132.70

Visit 2: Preparation of a GP Management Plan- (MBS Item 721 Fee: $138.75 Benefit: 75% = $104.10 100% = $138.75.), review and claim HMR (Item 900- Fee: $148.90 Benefit: 100% = $148.90)

Month 6

Visit 3: 3 months after visit 2, review GPMP (MBS Item 732 Fee: $69.35 Benefit: 75% = $52.05 100% = $69.35)

Visit 4, 3 months after visit 3, review GP Management Plan (MBS Item 732 Fee: $69.35 Benefit: 75% = $52.05 100% = $69.35)

Conduct a new Health assessment (annual MBS item 703) and refer for an annual HMR (MBS Item 900) and continue cycle as above

DVA Patients

The Department of Veteran Affairs (DVA) provides a  free Dose Administration Aid (DAA) service to eligible veterans who have difficulty managing their medicines. 

A Home Medicines Review is recommended as part of the process of assessing the veteran's suitability for the DAA service.

Step by step:

Once you have identified and obtained informed consent from a patient that may benefit from a Home Medicines Review (HMR), a referral must be generated and sent to the patient's preferred community pharmacy or can be sent directly to The Medicines Review Company. Contact us for the nearest pharmacy registered with The Medicines Review Company in your area.

Note: Changes to the HMR program commencing 1 October 2011 allow referrals to be sent directly to The Medicines Review Company. We still work very closely with the pharmacies registered with us to provide the best possible service and care to your patients.

Referrals can be in the form of a simple letter requesting a HMR for a patient or generated through templates often found in prescribing software. Ideally the referral should include relevant clinical information such as a current medication list, current medical conditions and recent pathology results and a reason for referral, highlighting any concerns that need particular attention.

Upon receiving the referral we will organise to visit the patient in their home. A HMR report is then produced containing suggested management strategies and any relevant information gathered at the interview.

The report is sent to the GP who then, after discussion with the pharmacist and patient, generates a Medication Management Plan. MBS item 900 can then be claimed.

We will send you reminder letters every 24 months when the patient is eligible for another Home Medicines Review.
